30-Day Failure Pre-Mortem: Field-Ready Prompt with 8 Risk Lenses - Digital Thought Disruption

TITLE: 30-Day Failure Pre-Mortem — Field-Ready Prompt (with 8 Risk Lenses) HOW TO USE (2 minutes prep) - Timebox: 20–40 minutes. Attendees do 5 minutes silent write, then round-robin. - Roles: Facilitator (keeps time), Scribe (fills template), Decision-Owner (can approve kill criteria). QUICK VERSION (60-second run) Imagine it’s 30 days from now and this initiative has failed. 1) What are the top three causes? 2) What early warning indicators (EWIs) would tip us off (with thresholds)? 3) What counter-moves do we put in place now (Prevent / Detect / Respond / Recover)?FACILITATION TIPS (use as needed) - Anchor discipline: silent write first prevents groupthink. - Force ownership: no counter-move without an owner and a date. - Gate to green: do not exit the meeting without kill criteria, rollback steps tested, and three assumption tests scheduled this week. The 30-Day Failure Pre-Mortem prompt gives teams a ready-to-use facilitation script for surfacing risks before launch. It forces concrete failure headlines, early warning indicators, counter-moves, and tested rollback paths. With the eight risk lenses, teams look beyond technology into governance, economics, sequencing, and human factors. Example: A team preparing a cloud migration uses the prompt. The 30-Day Failure Pre-Mortem prompt enables teams to move from vague concerns to actionable risk management in under an hour. By combining structured facilitation with eight risk lenses, it helps teams set clear thresholds, assign counter-moves, and validate rollback paths.

Why Startups Fail: A Look at Pre-Seed and Post-Seed Risks | by Inaki Berenguer | Medium

Why Startups Fail: A Look at Pre-Seed and Post-Seed Risks This memo explores the reasons startups fail at different stages based on my own experience as an entrepreneur and long-term angel investor … Interestingly, some YC companies from previous years that progressed within 20 months, reducing their risk profile for Seed or Series A funding, raised capital at valuations only slightly higher than their pre-seed demo day valuation. This indicates that initial high valuations often adjust to “fairer” valuations at later stages. So by investing later, you pay a little more, get a lot less risk, and avoid early stage failures. This memo explores the reasons startups fail at different stages based on my own experience as an entrepreneur and long-term angel investor (over 100 startups), focusing on some pre-seed and seed challenges.Selling Mission Critical Systems — Most preseed startups fail or pivot. This memo explores the reasons startups fail at different stages based on my own experience as an entrepreneur and long-term angel investor…

What Reasons Would You Fail a Pre-Employment Physical?

Learn What Factors Could Cause You to Fail a Pre-employment Physical at Express Health Urgent Care Serving Multiple Locations in NYC. A pre-employment physical exam is a requirement for most job offers, especially those that involve physically demanding tasks or require a certain level of fitness or health. The purpose of this exam is to ensure that candidates are physically and mentally fit enough to perform the tasks required by the job for everyone’s overall safety. What causes you to fail a pre-employment physical? There are several reasons why you may fail a pre-employment physical exam. These reasons include: • Cognitive or Mental Health Issues. Some jobs may require high levels of mental alertness or emotional stability. If you have cognitive or mental health issues that prevent you from performing the job’s required tasks, you may fail the exam.Jobs that require physical strength or endurance may require you to pass specific physical tests, and those who can’t meet the fitness requirements may fail the exam. • Substance Abuse. Drug and alcohol abuse can significantly impact your health and safety, which is why a pre-employment physical exam often includes drug and alcohol screening. If you have high blood pressure during a pre-employment physical, it doesn’t necessarily mean that you’ll fail the test. In most cases, the medical professional conducting the exam will inform your employer of your blood pressure readings and provide a recommended course of action.

prefailed, prepassed, failed, passed concepts in DEM - NI Community

Dear All, Currently i am studying diagnostic Event Manager(DEM) module and I have gone through concepts called prefailed, prepassed, failed and passed. As per my understanding prefail is used to count the error occurrence and stored in nonvolatile for the first operation cycle. The ones marked with '*' will keep the present Flag State. It is possible for the Pass, Fail, Fail, Pass, Fail states to continually show a Flag of . It is all dependent on the increment / decrement and count fault threshold. In the next operation cycle if the error is not present then the prepass counter used to maintain the status. But unable to get understanding about the Failed and Passed concepts.

How would you regard one who confidently agrees that he makes room for failure? He is probably not entrepreneurial or driven enough. We are taught to have a structured plan to focus on solely, which is the recipe for success. What if we prepare and have solid plans for failure, just like we have for success? One thing that we fail to realize is that success and failure are mere options. And we will inevitably have one. So why plan and prepare for one and not the other? “You either win, or you learn.” True. When we embark on any endeavor, we shut out every iota of thought, creeping into our consciousness to remind us of failure. We have several Gen8 and Gen9 servers and on all of them, when a physical disk has a pre-failure, the alert can be viewed in iLO (Storage Information). On Gen10 Servers I noticed that until today we haven't had this kind of pre-failure alert, what happens is the Virtual Disk on which the disk is mounted fails. Thanks for your help. Where in SSA do you see pre-failure info? How can you see if a drive is in a pre failure state on Gen 10 Servers? Does a prefailure get notified in the hard disk status in iLo now that SMH is not available on Gen 10 Server? Is there anywhere else to see HDD pre failures? So is there or isn't there a pre-failure alert for Gen10?

Why You Might Fail a Pre-Employment Physical in Australia | Comprehensive Guide

Discover the medical, fitness, and legal reasons candidates fail pre-employment physicals in Australia, plus practical steps to stay compliant and job-ready. Pre-employment physicals protect workers and employers, but uncontrolled health conditions, poor functional fitness, positive drug screens, or unmet industry standards can still derail job offers. This guide explains the most common failure reasons in Australia and shows candidates how to prepare while helping businesses remain fair and compliant.Even a history of a single seizure in the previous ten years may be disqualifying under Civil Aviation Safety Authority medical standards. In heavy-vehicle transport, the National Heavy Vehicle Regulator mandates seizure-free periods and specialist support before a commercial driver’s licence can be reinstated. A candidate can hold a perfect medical record on paper yet still fail the functional capacity component of an assessment.Functional testing varies by role: an underground fitter in Kalgoorlie might perform a repetitive lift-and-carry sequence with twenty-kilogram weights, while an aged-care worker in Hobart could demonstrate safe patient-transfer technique and sustained squatting. Failure occurs when the candidate cannot complete the series within the prescribed repetition count or time limit, or when clinical observation reveals unsafe biomechanics such as spinal flexion during lifts.Urine or saliva panels detect cannabinoids, amphetamines, cocaine, opiates and benzodiazepines, while breathalysers measure blood-alcohol concentration. A non-negative result typically means instant failure, withdrawal of the job offer and a mandatory stand-down period, because the presence of illicit drugs or hazardous levels of alcohol contravenes zero-tolerance policies in mining, maritime and aviation.

Why my tests fail only during pre-commit ? | by Mandar Vaze | Medium

Tests normally pass, but fail only during pre-commit only. I wonder why ? Why my tests fail only during pre-commit ? Symptom Recently I ran across (what I thought was) strange behaviour. I use pre-commit for all my git commits, and one of the step is to ensure that all the …During the pre-commit, pytest would return with exit code 5 - which indicates no tests ran. So it wasn't that one or more tests failed during pre-commit - but nothing was tested - and due to non-zero exit code, pre-commit prevented the commit (as it should)Each time when I tried commit, the unit test step in the pre-commit would fail - which indicates that the unit tests did not pass. But when I run make unit it would pass.I use pre-commit for all my git commits, and one of the step is to ensure that all the unit tests pass.
